Ask about reablement first, because it is free for up to six weeks. Then check the house, the stairs and the nights. Then ring providers. Do it in that order, because the free option disappears if you go private first.<\/span><\/p><p>\u00a0<\/p><h3><b>What &#8220;medically fit for discharge&#8221; actually means<\/b><\/h3><p><span style=\"font-weight: 400;\">It means the hospital has finished the part of the job hospitals do. It does not mean home is safe, and it does not mean anybody has looked at the house.<\/span><\/p><p><span style=\"font-weight: 400;\">Those two things get conflated constantly. Someone can be medically fit and still unable to get up the stairs, reach the bathroom at night, or open a tin.<\/span><\/p><p><span style=\"font-weight: 400;\">The other thing worth knowing: <\/span><b>the ward will usually give you a date around 48 hours ahead<\/b><span style=\"font-weight: 400;\">. That is the point at which most families start ringing round. Everything below is easier if you start before the date is confirmed.\u00a0<\/span><\/p><p>\u00a0<\/p><h3><b>Step one: ask about reablement, before anything else<\/b><\/h3><p><span style=\"font-weight: 400;\">This is the step that saves the most money and gets skipped the most often.<\/span><\/p><p><span style=\"font-weight: 400;\">Intermediate care and reablement are free for up to six weeks, under the Care and Support (Preventing Needs for Care and Support) Regulations 2014, and they are not means-tested (<\/span><a href=\"https:\/\/www.gov.uk\/government\/publications\/hospital-discharge-and-community-support-guidance\/hospital-discharge-and-community-support-guidance\"><span style=\"font-weight: 400;\">GOV.UK<\/span><\/a><span style=\"font-weight: 400;\">). Minor aids and adaptations up to \u00a31,000 must also be free.<\/span><\/p><p><span style=\"font-weight: 400;\">Ask the discharge coordinator, using these words: <\/span><b>&#8220;Has she been assessed for reablement or intermediate care?&#8221;<\/b><span style=\"font-weight: 400;\"> If the answer is no, ask why not and ask for the answer to be recorded.<\/span><\/p><p><span style=\"font-weight: 400;\">Most people receive one or two weeks rather than six, because reablement ends when the goals are met. Around half of people who complete it need no ongoing care at all, which is exactly why buying a long-term package on discharge day is usually premature.<\/span><\/p><p>\u00a0<\/p><h3><b>Step two: ask the questions the ward can answer and nobody else can<\/b><\/h3><p><span style=\"font-weight: 400;\">Write the answers down.\u00a0<\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><b>What is her weight bearing status?<\/b><span style=\"font-weight: 400;\"> Everything at home depends on this.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><b>Can she manage stairs?<\/b><span style=\"font-weight: 400;\"> Has anyone actually watched her try.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><b>What has changed on the medication list?<\/b><span style=\"font-weight: 400;\"> Ask for a copy of the discharge summary and read the changes.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><b>Has an occupational therapist assessed her at home<\/b><span style=\"font-weight: 400;\">, or only on the ward.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><b>What equipment is being delivered, and when?<\/b><span style=\"font-weight: 400;\"> A bed or a commode arriving after she does is a common and avoidable problem.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><b>Who is following up, and when?<\/b><span style=\"font-weight: 400;\"> District nurses, physiotherapy, the GP.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><b>Was a falls assessment done?<\/b><span style=\"font-weight: 400;\"> If she came in after a fall, NICE expects one.<\/span><\/li><\/ul><p>\u00a0<\/p><h3><b>Step three: walk the house before she comes home<\/b><\/h3><p><span style=\"font-weight: 400;\">A walk-through of the actual route she will take.<\/span><\/p><p><span style=\"font-weight: 400;\">Front door to a chair. Chair to the toilet, in the dark. Bed to the toilet at three in the morning. Getting the kettle down. Getting out of the bath. Where the phone will be.<\/span><\/p><p><span style=\"font-weight: 400;\">If the answer to &#8220;how does she get to the toilet at night&#8221; is a shrug, that is the thing to solve first, ahead of everything else on the list.<\/span><\/p><p>\u00a0<\/p><h3><b>Step four: work out what you actually need to buy<\/b><\/h3><p><span style=\"font-weight: 400;\">Once reablement is settled, the remaining question is what falls outside it.<\/span><\/p><p><span style=\"font-weight: 400;\">Reablement is about relearning tasks. It is not company, shopping, or overnight reassurance. Those gaps are what private care fills, and they are usually smaller than the package a frightened family buys at four o&#8217;clock on a Friday.<\/span><\/p><p><span style=\"font-weight: 400;\">Ask any provider three questions before signing: what is the minimum visit length, can we meet the caregiver first, and how does this reduce as she improves. A provider who cannot talk about reducing care is worth being careful with.<\/span><\/p><p>\u00a0<\/p><h3><b>Which hospital, and what that means<\/b><\/h3><p><span style=\"font-weight: 400;\">Discharges across this area come from four different places, and the teams differ.<\/span><\/p><p><span style=\"font-weight: 400;\">Macclesfield, Bollington, Prestbury, Sutton, Wilmslow, Handforth, Alderley Edge and Knutsford almost always go through <\/span><b>Macclesfield District General<\/b><span style=\"font-weight: 400;\">. Alsager and the south of the county come through <\/span><b>Leighton Hospital<\/b><span style=\"font-weight: 400;\"> in Crewe. Whaley Bridge and Chapel-en-le-Frith often route to <\/span><b>Stepping Hill<\/b><span style=\"font-weight: 400;\"> in Stockport. Kidsgrove and Biddulph generally go to <\/span><b>Royal Stoke<\/b><span style=\"font-weight: 400;\">.<\/span><\/p><p><span style=\"font-weight: 400;\">The council that picks it up depends on where she lives, not which hospital she is in. Someone discharged from Stepping Hill to a house in Whaley Bridge is Derbyshire&#8217;s responsibility, not Cheshire East&#8217;s, and that mismatch causes real delays if nobody spots it early.<\/span><\/p><ul><li style=\"font-weight: 400;\" aria-level=\"1\"><b>Cheshire East Council<\/b><span style=\"font-weight: 400;\"> adult social care, 0300 123 5010.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><b>Call Derbyshire<\/b><span style=\"font-weight: 400;\">, 01629 533190, for Buxton, Whaley Bridge and Chapel-en-le-Frith.<\/span><\/li><li style=\"font-weight: 400;\" aria-level=\"1\"><b>Staffordshire Cares<\/b><span style=\"font-weight: 400;\">, 0300 111 8010, for Leek, Biddulph and Kidsgrove.<\/span><\/li><\/ul><p>\u00a0<\/p><h3><b>The 48 hour version<\/b><\/h3><table><thead><tr><th><p><b>When<\/b><\/p><\/th><th><p><b>What to do<\/b><\/p><\/th><\/tr><\/thead><tbody><tr><td><p><b>As soon as discharge is mentioned<\/b><\/p><\/td><td><p><span style=\"font-weight: 400;\">Ask about reablement or intermediate care. Ask for the discharge summary and weight bearing status<\/span><\/p><\/td><\/tr><tr><td><p><b>Same day<\/b><\/p><\/td><td><p><span style=\"font-weight: 400;\">Walk the house. Bed, toilet, stairs, kettle, phone. Note what is impossible rather than what is untidy<\/span><\/p><\/td><\/tr><tr><td><p><b>Same day<\/b><\/p><\/td><td><p><span style=\"font-weight: 400;\">Check equipment is ordered and confirm the delivery date<\/span><\/p><\/td><\/tr><tr><td><p><b>Next day<\/b><\/p><\/td><td><p><span style=\"font-weight: 400;\">If private care is needed on top, ring two or three providers. Ask about minimum visit length, meeting the caregiver, and how care reduces<\/span><\/p><\/td><\/tr><tr><td><p><b>Next day<\/b><\/p><\/td><td><p><span style=\"font-weight: 400;\">Sort the practical basics: food in the house, heating on, a key safe, someone doing a first night check<\/span><\/p><\/td><\/tr><tr><td><p><b>Within a week of getting home<\/b><\/p><\/td><td><p><span style=\"font-weight: 400;\">GP appointment. Medication review, and a falls assessment if she came in after a fall<\/span><\/p><\/td><\/tr><\/tbody><\/table><p>\u00a0<\/p><h3><b>If the discharge feels too early<\/b><\/h3><p><span style=\"font-weight: 400;\">Say exactly that, to the discharge coordinator, and ask for your concern to be recorded in the notes.<\/span><\/p><p><span style=\"font-weight: 400;\">Be specific rather than general. &#8220;She cannot get up the stairs and the only toilet is upstairs&#8221; carries weight. &#8220;I&#8217;m worried&#8221; does not, however true it is. If you cannot get anywhere, ask to speak to the ward manager or the hospital&#8217;s patient advice and liaison service.<\/span><\/p><p><span style=\"font-weight: 400;\">Being honest about the other side: wards are under real pressure, and a longer stay is not automatically safer. Time in hospital deconditions older people quickly. The aim is a safe discharge, not a delayed one.<\/span><\/p><p>\u00a0<\/p><h3><b>Frequently asked questions<\/b><\/h3><p><b>How quickly can care be arranged?<\/b><\/p><p><span style=\"font-weight: 400;\">Often within a few days of an initial visit, sometimes faster. What takes longest is usually not the provider but the assessment and equipment, which is why starting before the date is confirmed matters.<\/span><\/p><p><b>Do we have to accept the provider the hospital suggests?<\/b><\/p><p><span style=\"font-weight: 400;\">For reablement, yes, that is arranged for you. For ongoing care you are paying for, no. If the council is funding it, a direct payment lets you choose and top up.<\/span><\/p><p><b>Can she be discharged if we say we cannot cope?<\/b><\/p><p><span style=\"font-weight: 400;\">Family cannot be required to provide care. Say clearly and early what you can and cannot do, because a plan built on an assumption nobody checked is the commonest cause of a readmission.<\/span><\/p><p><b>What if she is discharged at nine at night?<\/b><\/p><p><span style=\"font-weight: 400;\">It happens. Make sure the house is warm, there is food and a drink to hand, the phone is by the chair, and someone checks in the following morning. Sort the rest the next day.<\/span><\/p><p><b>Will she need care permanently?<\/b><\/p><p><span style=\"font-weight: 400;\">Often not. Many packages after a hospital stay run six to twelve weeks and then stop or reduce. Agree a review date at the start.<\/span><\/p><p><b>Who pays while we wait for the council assessment?<\/b><\/p><p><span style=\"font-weight: 400;\">If you start privately, you pay.
